Transforming Health Care for 28 Million Members---Supports care management activities and ensures services are delivered by healthcare providers and partners for continuity of care/member satisfaction.--------- Provides outreach to members via phone or home visits to engage and discuss care plans/service plans--------- Coordinates care activities based on care plans/service plans and works with healthcare and community providers and partners--------- Serves as support on member and/or provider inquiries, requests, or concerns related to care plans/service plans--------- May support performing service assessments/screenings for members and documenting care needs------ Follows standards of practice and policies compliant with contractual requirements and regulatory guidelines and standards------ Requires a High School diploma or GED and 1-2 years of related experience------ Pay Range: $17.17 - $26.97 per hour, competitive benefits, and flexible work schedules------ Centene is an equal opportunity employer committed to diversity and values differences.
