Job Description:
A company is looking for a Panel Assistant to contribute to the
medical home model of care by managing and coordinating patient
care.
Key Responsibilities
Assist the clinic care team in managing patient care and
coordinating wellness screenings
Monitor patient attribution, schedule office visits, and manage
chronic disease registries
Collect and communicate patient data to the healthcare team and
follow through on care plans
Qualifications
Post-secondary education of one to two years in a healthcare or
medical-related field preferred
Background as a Certified Medical Assistant (CMA) is preferred
