Key responsibilities include:

• Coordination of the total plan of care with other healthcare professionals involved in care, ensuring continuity of patient care by planning and exchanging information with the physician, agency personnel, patient, family, and community resources within a Point of Care setting.
• Initial and comprehensive nursing evaluation visits, determining the primary focus of care, developing the plan of care within state-specific guidelines, and submitting accurate, complete, and timely documentation in collaboration with physician orders.
• Patient comprehensive assessments at designated time points and development of the appropriate plan of care.
• Ensuring patients meet home health eligibility and medical necessity guidelines as defined by the payer source.Required qualifications include:
  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
