Job Description:

A company is looking for a Market Registered Nurse to provide virtual clinical care and support for patients.
Key Responsibilities

Deliver compassionate, individualized patient care and education virtually
Conduct telephonic triage for urgent and emergent patient needs
Coordinate care and develop individualized action plans to improve clinical outcomes

Qualifications

Bachelor's degree in Nursing or equivalent clinical experience
Active nurse licensure in New York, in good standing
Recent experience in telephonic triage, urgent care, or ER/ED settings
Clinical experience with aging adults and complex care needs
Advanced training in geriatrics and/or palliative medicine preferred
