Job Description:
A company is looking for a Care Coordination Nurse II-RN
(Part-time).
Key Responsibilities
Respond to incoming and outgoing calls regarding clinical issues
and patient care
Evaluate patient referrals for appropriateness for home care and
document necessary information
Provide telephonic health information and nursing education to
patients
Required Qualifications, Training, and Education
Graduate of a US accredited nursing program with an active RN
license in good standing
Minimum of 5 years of home health experience
Knowledgeable about home health benefits and medical necessity
requirements
Experience in home health, emergency department, or case management
preferred
Must possess current licensure as a Registered Nurse in the state
of practice
