Job Description:

Responsibilities:

• Assess and develop therapeutic retraining programs for patients in their homes
• Confer with the patient's physician and healthcare team to develop treatment plans
• Provide therapeutic treatment to improve strength, coordination, and function
• Review and expand retraining programs in line with patient goals
• Consult with vocational team members to develop vocationally oriented plans
• Report patient status and progress to relevant parties
• Participate in care coordination and discharge planning
• Design community reintegration activities
• Recommend and/or design special adaptive equipment for clients
• Train staff and family members to carry out retraining programs Requirements:
  • Degree from an accredited Occupational Therapy Program
  • A minimum of six months of occupational therapy experience preferred
  • Home Health experience is a plus
  • Current and unrestricted OT licensure
  • Current CPR certification
  • Good organizational and communication skills
  • Valid driver's license, auto insurance, and reliable transportation
